The anal veins draw blood away from the anal canal and the hemorrhoids and drain in two ways.  One direction is upward into the rectum and the other direction is down through the skin that surrounds the anus.

If the anal canal has a hemorrhoid form at the top or inside, it is called an  Internal Hemorrhoid  If the hemorrhoid forms near the anus, in the anal canal, it is called an External Hemorrhoid The only real difference between an Internal Hemorrhoid or and External Hemorrhoid is its position above or below the "Pectinate Line" (which is a line that divides the upper 2/3rds and lower 1/3rd of the anal canal.

 When the hemorrhoids are forming, the blood vessels and the supporting tissues swell.  There are then bulges that protrude into the anal canal making it extremely uncomfortable and also painful.  It has not yet been determined the reasons for the formation of External Hemorrhoids.
